Would you like to sign up for the COVID-19 Vaccine?
Currently the Greene County Health Department is following the Center for Disease Control (CDC), Illinois Department of Public Health (IDPH), and the Advisory Committee on Immunization Practice’s guidelines. You can find more information about these guidelines and the different phases on the Illinois Department of Public Health’s webpage located here.

Once you submit this form you do not need to resubmit the form more than once. We will continue to work through the phases as directed by the CDC, IDPH, and ACIP. Our staff will reach out to you once we have the vaccine available for your group to schedule your appointment.
Please be patient and allow for up to 6-8 weeks before you are called to be scheduled as vaccine supplies are limited and this will take some time to work through the different phases. Please do not show up at any vaccine clinic that you are not scheduled for as our staff will be unable to assist you.
