Fee includes Classroom Review, and Exam.
Note: Pre-study of the current Food Code is required prior to class date. The GCHD has the 2022 FDA Food Code by ServSafe, a pre-study textbook available to use for a $55.00 deposit. Upon return of the book (undamaged) the deposit will be returned.
- Illinois Department of Public Health (IDPH) regulations require that Certificate candidates MUST complete an approved 8-hour Food Protection Manager Course.
To Attend Class: Enrollment and payment MUST be received before specified date for class to have necessary class materials available. A photo ID must be presented at class – valid Driver’s License, Secretary of State ID card, school ID card, firearm owners ID (FOID) card only if signatures and photo are shown, military ID card or a current passport are acceptable as photo identification.
Courses can be paid by cash, check, major credit, debit and prepaid debit card brands. If paying with a credit or debit card, payment can be made on-site or on-line. Go to www.govpaynow.com. Enter the Pay Location Code (PLC) #7912 and complete requested information. Please be aware fees do apply.
