The Illinois Department of Human Services and Illinois Department of Public Aid fund the Family Case Management Program. It provides services for pregnant women, infants, and high-risk children, as well as pregnancy testing. The objective of the program is to provide age appropriate client education, risk assessment, development of an individual care plan, referral to appropriate providers and advocacy to assist participants in accessing services; thereby enhancing a favorable health outcome. Services are provided in the office and home setting.

Any client exhibiting risk factors is referred to their physician for follow-up care. Home visits are provided for high-risk children for assessment and referral if necessary.
