You can arrange home health services by asking your Doctor to call the Greene County Health Department directly. A registered nurse will assist you in making the necessary arrangements. An evaluation home visit is available as needed.

Greene County Health Department offers the following Home Health Services:
Skilled Home Health Services
Skilled Home Health Services may include one or more of the following services:
- Skilled Nursing
This service provides part-time or intermittent nursing care as ordered by the patient’s physician for homebound patients due to disease or disability. Coordinated discharge planning with the hospitals is a valuable tool to ensure continuum of care to the citizens of Greene County. The Department serves both Greene & Scott County. In addition to our skilled nursing program, patients can receive one or more of the following additional services:
- Home Health Aide
Services such as bathing, personal care, exercises, ambulation, and respite are some of the services provided to homebound patients by the Home Health Aide. Services are provided under the supervision of a Registered Nurse in accordance with the patient’s Plan of Care. This program provides a key service to the citizens of Greene County and enables them to remain in their homes thus avoiding premature institutionalization.
- Therapy Services
Occupational, Physical and Speech Therapy are also offered through a contracted provider. Occupational Therapy provides training in independent living skills, muscle re-education, adaptive equipment, splinting/orthotics, fabrication and training. Physical Therapy provides therapeutic exercises and gait training for the patient who has suffered a stroke, fracture, traumatic injury, etc., Speech Therapy services those patient’s who have lost their ability to speak or suffer a swallowing disorder.

- Most Private Insurance

In-Home Respite Services
Respite services are provided to the caregiver to help provide temporary relief of care and supervision on functionally impaired persons age 60 and older.
In order to be eligible for respite care services, family caregivers must be providing in-home and community care to older individuals (age 60 or more) who meet the following definition of frail:
- Is unable to perform at least two activities of daily living without substantial human assistance, including verbal reminding, physical cueing, or supervision; or
- Due to a cognitive or other mental impairment, requires substantial supervision because the individual behaves in a manner that poses a serious health or safety hazard to the individual or to another individual.
Funded by Area Agency on Aging for Lincolnland.

Access Assistance Program
The Access Assistance Program provides a starting point for services for caregiving families.
This program provides access to information, training, support groups, counseling, resource libraries, respite care and supplemental services to family caregivers and to grandparents raising grandchildren. Follow-up will be conducted to see if needs were met.
How to figure out your role as CAREGIVER-
- I provide regular emotional or physical assistance (such as helping with lifting or helping someone move from a bed to a chair) to my spouse, parent, neighbor, adult child or friend.
- I help a senior pay bills.
- I shop or do errands for a senior.
- I drive a senior to the doctor or take him/her to a community center.
- I remind a senior to take pills or provide help with personal care.
- I regularly prepare meals or clean house for a senior.
