Our Community Health Division works to build individual and community capacity by increasing self-sufficiency and health knowledge, also known as health literacy, through a range of activities such as outreach, community education, informal counseling/support, and social support.  This work is accomplished through our community health workers and our recovery support specialist.

Though you don’t hear the term health literacy that often, it is a critical piece of your health.  By improving our health literacy we can help prevent health problems and protect the health of our community, as well as better manage problems and unexpected events.   Our Community Health Division is committed to ensuring that you have the latest, most accurate information you need to protect your health!
