COVID-19 VACCINATION CLINIC

Everyone ages 5 & older is eligible to receive the COVID-19 vaccination.  Children under 18 years old must be with a parent or legal gaurdian at the time of immunization or send a signed notarized permission note with another adult for the minor child.
