Health Education Program
The Community Health team provides a comprehensive health education program that works to meet the needs of everyone we serve in our community.
Our Community Health Workers or (CHWs) aim to provide:
- Diabetes, cancer, and other Chronic disease self-management education
- Provide education and information on preventing disease and the spread in our community through handwashing, immmunizations, preventative screenings, and other healthy habits
- Work to improve and enhance enviornmental health by educating residents on Fight-the-Bite, vector/rodent control, childhood illness prevention such as lead, food safety, and much more!
- Provide personal preparedness education to ensure our community is ready for emergencies
- Community outreach/engagement

Community health workers (CHWs) play a vital role in promoting and improving the health and well-being of individuals and communities, especially when it comes to health education. Our team represents the communities they serve and are trained to provide a range of health-related services and support. Here’s a comprehensive description of what our community health workers do:
- Health Education and Promotion: CHWs educate community members about various health issues, including preventive measures, healthy lifestyle choices, and disease management. They conduct workshops, seminars, and outreach programs to disseminate information on topics such as nutrition, hygiene, family planning, and immunizations.
- Advocacy and Outreach: CHWs advocate for the health needs and rights of their communities. They work to identify health disparities, access barriers, and social determinants of health, advocating for policies and resources to address these issues. They also conduct door-to-door outreach to connect individuals with healthcare services and resources.
- Health Screening and Assessment: CHWs conduct health screenings and assessments to identify individuals at risk for various health conditions. This may include measuring vital signs, conducting screenings for chronic diseases such as diabetes or hypertension, and assessing mental health needs. They then refer individuals to appropriate healthcare providers for further evaluation and treatment.
- Case Management and Referrals: CHWs provide ongoing support and guidance to individuals with chronic health conditions or complex health needs. They help clients navigate the healthcare system, schedule appointments, obtain medications, and access social services. CHWs also facilitate communication between clients and healthcare providers to ensure coordinated care.
- Cultural Mediation and Language Support: CHWs bridge cultural and linguistic barriers between healthcare providers and community members. They understand the cultural norms, beliefs, and practices of their communities and use this knowledge to deliver culturally sensitive care. CHWs may also serve as interpreters or translators, ensuring that individuals understand medical information and can communicate effectively with healthcare providers.
- Community Mobilization and Capacity Building: CHWs work to mobilize community members and build their capacity to address health issues collectively. They facilitate the formation of community health committees or support groups, empowering individuals to take ownership of their health and advocate for change within their communities. CHWs may also collaborate with local organizations and stakeholders to develop community-driven health initiatives and programs.
- Data Collection and Reporting: CHWs collect data on community health indicators, such as disease prevalence, health behaviors, and access to healthcare services. They use this information to identify trends, monitor the effectiveness of interventions, and inform decision-making at the community level. CHWs may also assist in conducting research studies or health assessments to gather more comprehensive data on community health needs.
Our team of community health workers play a multifaceted and vital role in promoting health equity, empowering our communities, and improving access to healthcare services throughout our region. Through education, advocacy, and direct support, they serve as trusted allies and advocates for healthier communities.
